The Hamilton Family Medicine Primary Care Network — a collaboration of stakeholders developing the care hub at the centre on Wentworth Street North — will be finalizing their model in January, deciding on how the hub will operate, including access to family doctors.
The new Health Care Hub will provide family medicine and services for mental health and nutrition. “We learned that people in the area are going to the emergency rooms when they have acute needs — trips that can be prevented by comprehensive, full-scope, team-based primary care,” said Dr. Brian McKenna, the project lead and Hamilton Family Health Team’s deputy lead physician.
Our speaker will be Anne Howarth, providing an update and details about this new innovative approach to heath care in the Keith Neighbourhood Hub at the Eva Rothwell Centre (Former Robert Land School)
